Forms to Complete
The following forms are used so that the testing is carried out in accordance with our accreditation criteria. This makes sure that the result is correct and legally valid. Please download the required forms and complete with your details. The Instructions for DNA testing form is provided to help you through the process. However, if you have any queries please do not hesitate to contact us before entering your details.
Client Information Sheet

Please complete all required fields, including the address that results should be sent to.

If a lawyer represents a client, these details are required on this form and results will be sent to them.

Details regarding payment must be completed.
All forms must be taken with you to your appointment, so that our staff may enter the relevant details and that the identity of each person sampled can be verified. Forms will be kept by the laboratory while samples are analysed, and then will be stored securely for future verification.
Form 2: Parentage Testing Procedure Affidavit by/In Relation to Donor

Must be completed and witnessed by a Justice of the Peace on the same day, just before your appointment - Part I for all adults and Part II for children.

Each adult client must complete their own Form 2 (regardless of whether participant is 'child' or 'assumed parent').

Any participant 18 years or under, must have a Form 2 completed by their Legal Guardian (regardless of whether participant is 'child' or 'assumed parent').

Each participant will require a passport sized photo to be attached to Form 2.

Please use a second Form 2 if a second child is being tested.
Form 4: Parentage Testing Procedure-Collection of Bodily Samples

The person who takes the samples will complete this form at the appointment (so it must be taken with you to the appointment).

Each person having a sample collected at that appointment must provide a passport sized photograph (recent, clear head and shoulders colour photograph).

The provided photographs will be attached to the form and will be signed by the collector to verify that the person in the photograph is the same person the sample was taken from.

If there is a dispute about the result of the test, the photograph may be viewed* to verify the identity of the person who gave the sample.
*At the discretion of laboratory staff unless accompanied by a court order.
What it costs
As a guide, the cost to test presumed father, mother and child would be $770 (including GST). This cost includes the necessary forms, sample collection and transfers, laboratory analysis and the production of a NATA accredited report.
